Teacher Forms
To submit student scheduling updates for spring semester, click YOUR NAME from the list below to access the assigned document for your students. Inside your assigned document, select tab labeled “Student Status Report” to complete the required forms.
NEW STUDENT/Lesson Time Form
Once you confirm the new student lesson time with the parent, complete the online NEW STUDENT FORM by clicking HERE. After you complete the online form, DMA Administrators will add the student to your online attendance calendar & your online schedule tab.

Spring Schedule Forms
Student Report 1 / Due December 5
Click on tab labeled “Student Status Report.” Complete each question included in the yellow highlighted box titled “Spring Schedule Info.”
Student Report 2 / Due December 19
Click on tab labeled “Student Status Report.” For each returning student listed, select the dropdown option that indicates each student’s current scheduling status as of December 19.
“Schedule Freeze” Window / January 1-15
A “Schedule Freeze” window is in effect for current students from January 1-15, which means that no schedule changes can be made during this timeframe. If there are extenuating circumstances, contact Kimberly prior to agreeing to any schedule change requests made by parents to confirm what options might be available.
New Student Match Day / January 7
Teachers will receive email notification that new students have been added to their assigned google doc.
Spring Semester Begins / January 11
