To submit summer scheduling forms for your students, click YOUR NAME from the list below to access the assigned document for your students. Click on the designated tab inside your assigned document to view various scheduling information.

NEW STUDENT/Lesson Time Form

Once you confirm the new student lesson time with the parent, complete the online NEW STUDENT FORM by clicking HERE. After you complete the online form, DMA Administrators will add the student to your online attendance calendar & your online schedule tab.

Summer Schedule Form / Due June 3

Click on tab labeled “2025 Summer Schedule” For each returning student, type student’s FIRST and LAST name into the correct day and time on your schedule. On your schedule form, be sure to include student’s lesson start time and end time. Also, double check that the lesson length indicated on the “Registered Students” tab matches the student’s lesson length on your schedule.

Summer Registration List

Click on tab labeled “Registered Students” to view the list of summer registrations. In this tab, you will see each student’s name, lesson length, and number of lessons for summer session.

NEW STUDENTS/Parent Contact Info

When new students are assigned to you, new student scheduling information will be added to the “Registered Students” tab. Additionally, new student contact information will be added to the “Parent Contact Info” tab.

Scheduling Deadline

Summer schedule forms are due Tuesday, June 3. All forms will be considered “COMPLETE” after that date, which means you will no longer be able to make changes to student lesson times on the shared document.

Once you finalize all lesson times for returning students, be sure to scroll to the top of the document and click “SCHEDULE FINALIZED” to record your responses.

After the submission deadline on June 3, you will still be able to VIEW your document information, but you will no longer be able to EDIT the shared document.
